一辆救护车与神秘的跑车“地狱猫”从港口、山道一路厮杀到高速公路，城市之中仿佛武当之巅高手过招，到底新一代的车神会是谁？如果两个只能活一个，谁人笑到最后？开着救护车的是“小乐”（瘦子饰）一个曾经的最佳赛车手。小乐在一场与哥哥“阿杰”（柯有伦饰）的比试中，让哥哥意外身亡，小乐自暴自弃，直到遇见女医生“慧慧”（Ella陈嘉桦饰），转念成为了救护车司机，誓言要“用速度救人”。然而一名神秘赛车手“地狱猫”的主人突然现身，他手握名册，被挑战的赛车手最终必然丧命……地下赛车，就是一场现代武林，尽管以生命作为代价，仍无法阻挡狂热者对速度进行宿命般的追逐。在小乐的死党“三宝”（大渊、林真亦、林志谦饰）的调查下，各个意外的真相将逐渐浮现……
